Developmental Services - Housing Stability Case Manager (Prevention & Shelter Diversion)

One Full-Time Contract to March 2029 (with the possibility of extension or becoming permanent) 

 
  • Location: Aurora, Ontario L4G3S9 
    • must be within a commutable daily distance from the office and/or service area 
  • Status: Contract, Full-Time
  • Starting Hourly Rate: $32.13 / Hour
  • Office Hours: Monday - Friday - 8:30 am - 4:30 pm, 35 Hours per week
  • Office Expectations; Work in the office to provide team coverage, minimum once per month
    • in addition to in-office meetings, trainingssupervisions and all-staff events

Job Description


The Housing Stability Outreach Worker provides front-line homelessness prevention and shelter diversion supports for people in York Region who are experiencing homelessness or are at imminent risk of losing housing (within 30 days). The role delivers mobile, community-based outreach; completes housing stabilization and diversion planning; coordinates short-term emergency assistance; and connects people to crisis services, mental health supports, developmental services, income supports, and other community resources to support sustainable housing outcomes.

Working Conditions

  • Community-based role with travel across York Region; work may occur in agency offices, community settings, hospitals/public institutions, and partner locations.
  • Ability to work with individuals experiencing crisis and complex needs, including exposure to challenging situations.
Position Requirements
  • Bachelor’s degree in Social Work or related discipline (i.e. Health Sciences)
  • Experience;
    • Minimum three years related experience
    • Providing community-based outreach, housing stabilization, homelessness prevention/diversion, or intensive case management
    • Demonstrated knowledge of mental health, developmental disabilities, and the impacts of trauma, poverty, and housing instability
    • Strong skills in advocacy, service navigation, and coordinated care planning with multiple partners.
    • Excellent interpersonal communication skills, including de-escalation and conflict resolution.
    • Ability to maintain accurate documentation and work with data/outcomes tracking requirements.
    • Working within a coordinated access/homelessness-serving system and with prevention & shelter diversion funding/program models.
    • Supporting hospital discharge planning or transitions from public institutions.
    • Knowledge of harm reduction practice and community-based substance use supports.
    • Familiarity with landlord-tenant systems and local housing resources.
    • Second-language proficiency and/or experience providing accessible, low-barrier services across cultures.
  • Second language is considered an asset
  • An understanding of cultural diversity is considered an asset

*   A valid G Drivers License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation.
