Description
One Full-time Contract to March 31, 2025 (with the possibility of extension or becoming permanent)
Streamlined Access provides coordinated access to 26 different mental health, addictions and dual diagnosis services (mental health and developmental disability) in York Region and South Simcoe.
The Mental Health Access Worker is responsible for the provision of a centralized access response for adults with a serious mental illness, addiction and/or dual diagnosis to the mental health system. The Access Worker conducts psychosocial assessments to determine the support needs of individuals who are experiencing symptoms of mental health, addictions, or a dual diagnosis. Once the support needs of the individual are determined, the Access Worker will connect the individual to services and make outbound referrals for services not accessed through Streamlined Access.
The Access Worker will liaise with outreach workers, hospital social workers and crisis workers, shelters, primary care providers, municipalities, emergency services and community mental health workers, to support individuals on accessing the supports requires for recovery.

Position Requirements

Office Hours: Monday - Friday - 8:30 am - 4:30 pm
Office Expectations: 1 staff in daily for team coverage, meetings, and trainings when required.

Position Requirements
- Bachelor degree in Social Work or a related discipline (ie Psychology etc.)
- Registration with a professional body is required (ie Ontario College of Social Workers and Social Service Workers)
- Ability to work flexible hours, including evenings is required
- A minimum three years related experience required
- Counseling, crisis prevention and intervention experience
- Demonstrated time management, organizational, interpersonal and communication skills
- Experience working with individuals having serious mental illness is essential
- Broad experience with individual, family and systems advocacy
- Proven ability to exercise superior judgement
- Demonstrated change management skills and ability to adapt to the ever-changing nature of the Social Services sector
- Experience with assessment and prioritization tools such as Locus, Gain SS, VI-SPDAT and OCAN are considered an asset
- Francophone or second language is considered an asset
- The successful applicant will be required to provide a police clearance that meets agency standards
- A valid G driver’s license, business use insurance, and reliable vehicle are required for this position.
