Mental Health Case Manager - Flexible Supports

One Full-Time Contract position to June 2027 (with the possibility of extension or becoming permanent)

 
  • Status: Contract, Full-Time
  • Starting Hourly Rate: $32.13 / Hour
  • Office Hours: Monday - Friday - 8:30 am - 4:30 pm, 35 hours per week
  • Location: Aurora, Ontario L4G 3S9 
  • Office Expectations; This position requires:
    • A minimum of one in-office day per month to provide team coverage
    • Attendance at additional in-office meetingstraining sessionssupervision meetings, and all-staff events as required
    • Applicants must reside within a reasonable daily commuting distance of the office and/or assigned service area and be able to attend the office as required for operational needs

Job Description


This position will provide intensive case management and advocacy services to individual’s aged sixteen and over who are experiencing mental health, dual diagnosis and/or addictions and who are vulnerably housed or homeless. Through a collaborative approach, this position will facilitate recovery and assist service users who are experiencing significant difficulties in daily functioning to improve their quality of life. Tasks will include direct service interventions, clinical support and community development. Daily travel within York region will be required. 


Position Requirements
  • Bachelor’s degree in social work or related discipline (i.e. Health Sciences)
  • Experience;
    • Minimum 3 years related experience in social services with adults who have a serious mental illness, dual diagnosis, developmental disability, or addictions 
    • Broad experience with individual, family, and systems advocacy, supportive counseling, crisis prevention and intervention experience 
    • Experience in providing support around housing and/or homelessness 
    • Sound understanding of the population and relevant legislation 
  • Second language is considered an asset
  • An understanding of cultural diversity is considered an asset
*   The successful applicant will be required to provide a police clearance that meets agency standards
*   A valid G Drivers License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation.
